What is laparoscopic IPOM?

Laparoscopic IPOM refers to the intraperitoneal onlay mesh procedure that involves the direction of the bowel out of the hernia sac via small keyhole incisions under laparoscopic vision. The placement of mesh over the hernia opening makes use of absorbable tackers, thus closing the defect. The mesh is a special mesh that is covered by a special lining, preventing adhesions. However, you may form adhesions; this is variable and will be dealt with appropriately.

Laparoscopic Intraperitoneal Onlay Mesh (IPOM) is an effective surgical technique for specific types of hernias, more specifically, ventral hernia repair and has been adapted for inguinal hernias in certain situations. It involves the surgical placement of the mesh directly within the peritoneal cavity, overlying the hernia defect, with the peritoneum left as is.

The IPOM technique is based on “tensionless repair,” in which the hernia is concealed with a prosthetic mesh rather than relying on a suture-based repair. The mesh is inserted intraperitoneally and secured firmly to strengthen the weakened abdominal wall and minimise the risk of recurrence. The mesh makes contact with the intra-abdominal organs; in such cases, a composite anti-adhesive mesh is the staple. As a result, it reduces complications such as fistula formation, bowel obstruction, and adhesions.

How do you prepare?

A routine preoperative evaluation is required, including a complete laboratory workup, physical examination, and history-taking. Imaging studies, such as a CT scan or ultrasound, are often mandatory for complex or recurrent hernias. Usually, the bladder is emptied before the procedure.      

What’s recovery like?

You will need to wear a brace for three to six months and avoid heavy lifting for three months. Recovery from laparoscopic IPOM repair is fairly quick with minimal pain compared to traditional open surgery.
